Medicare, Medicaid, Highmark Blue Cross/Blue Shield insurances, UPMC, Tricare, UMWA, Unison, Medical Assistance/Access Plus, Gateway/Medicare Assured, Aetna and commercial insurances cover the cost of home care visits. Eligibility Requirements
Individuals must be essentially homebound, requiring skilled care and under a physicians care. "Homebound" means that leaving the home is a taxing effort; the patient needs the assistance of medical equipment, such as a cane, walker, wheelchair, and/or personal assistance at times to manage things such as oxygen or transportation. Patient may leave the home for short periods of time to go to the store, physician, church, etc. **If the patient is able to drive themselves to a doctor appointment safely, they are considered NOT homebound** Please call us for more information and/or explanation.
Required Documentation
Physician's order for a home health evaluation. Physician's order can be from the primary care physician, specialist (ex. cardiologist), nurse practitioner, physician assistant. Contact us for more information and/or explanation.
